Yeah generally rulebreaking all around.

The MFK I could see being as just confusion, but it's almost remarkable in that the only three killed were all not KOS which makes me believe it was intentional.
Coupled with everything else and the warnings I gave him in-game beforehand myself, I'm calling the MFK intentional.
