Let me help you out buddy... Here on panda we got Hlstatsx, which you can navigate to here by clicking on "stats". There you can find all of our servers including jailbreak, and by clicking on jailbreak you can later on click on "players" and search for KingDuckYT and get his SteamID and chatlog (aka the evidence). So your report would look something like this then:
